They sit at the natural waist over the top of the kilt rather than through any loops, because a kilt has none. Size to your kilt waist plus two inches, since the belt has to clear the pleats and the apron. The buckle is the expensive part and the strap is the simple part, since the strap carries no holes and no prong and is gripped by the buckle itself. If a kilt no longer fastens comfortably, an extension strap adds a few inches at the existing buckle and costs a fraction of alteration or replacement.
